Get some foam tips

These foam tips really changed the comfort on my IEMs. I went from not being able to do more than 1 hour to being able to wear them for multiple hours with no ear-canal pain.

Try them out if you've only used silicone tips before ;)

These are excellent ear tips and the only ones I would buy for my IEMs.

The only problem is they don’t last as long as I’d like them to, even with cleaning and following the Comply care tips. That being said, they allow me to listen at a much lower level than with other ear tips without such good sound isolation.
